it is just the cowling that encases the engine. If it wasnt fastened correctly, would very likely happen. Doesn't mean the engine is faulty or that the plane is faulty, just highly skilled, highly trained, highly paid mechanics did a great job!

the last thing that's supposed to happen is the pilot(s) are supposed to do a walk around their aircraft before takeoff..... yes, line maintenance didn't fasten the cowling properly, and the pilot(s) missed it on their walk around, or just don't do a walk around at all.....
