Two mistakes:
1. Faulty torpedo fired against the enemy, not missile accidentally launched at Trinidad herself.
2. And this particular incident didn't kill her. HMS Trinidad reached safe harbour, where she underwent some repairs. She sunk later, in other circumstances (damaged beyond repair by German bomb, then torpedoed by one of the escorting ships).

#5 during that period of time, torpedos were not driven into enemy ships or subs, there were many instances of the weapons turning on the ship that launched the weapon, the weapon did not have the circuitry or sophistication to recognize the launch or friendly ships (called ASH).
